Migrate legacy form

Upload a legacy compiled-form JSON and its PDF, choose a destination folder, and migrate it into a new form. The form's stable id (UUID) is taken from the JSON, so a later deploy updates the original document.

What gets migrated

Fields, rules, signatures, supporting documents, strikethroughs, and entity IDs are all carried over. The PDF is attached as the form's current document. Re-migrating a JSON with the same id overwrites the existing form in place.
